MyChart Terms and Conditions of Use

MyChart is provided by the Board of Trustees of the University of Illinois, University of Illinois Hospital & Health Sciences System (UI Health) to its patients subject to the following terms and conditions. Please read the following information carefully

By accessing MyChart, you are agreeing to abide by and be bound by these terms and conditions of use (Terms). Agreeing to these Terms is voluntary and your treatment or eligibility for third party payment coverage is not conditioned upon agreeing to these Terms. If you do not agree to the current Terms, you may not establish or use MyChart.

UI Health Response to Electronic Communications. MyChart should never be used for urgent medical matters. If you have any urgent medical issues, please call your healthcare provider’s office. In an emergency, go to your nearest emergency department or call 911.

For non-urgent matters, UI Health will attempt to provide timely responses to electronic communications sent via MyChart. If your healthcare provider is out of the office or unavailable, your MyChart messages may be routed to another authorized healthcare provider. Sometimes, the member of your health care team needed to provide the response to your MyChart message may not be immediately available, and so you should allow at least two (2) business days for a response. UI Health healthcare providers respond to your MyChart messages based on the information that is provided to them; therefore, if insufficient information is provided, they will be unable to provide accurate or reliable services.

Privacy of Electronic Communications. You will receive e-mails to notify you that new information is available to be viewed in your MyChart account. The e-mail notifications will be sent to your e-mail account and will not contain confidential health information. Any person with access to your e-mail account, however, will be able to see the notification e-mails.

MyChart Eligibility and Proxy Access. Children under the age of 12 may not establish a MyChart account. Adolescent minors between the ages of 12 to 17 may request individual access to their own MyChart account. Parents or guardians may request access to their minor child’s account. State and federal laws, however, restrict certain types of information that the parent of a minor child above age 12 may access without the consent of the minor. Therefore, when a minor patient turns age 12, only limited medical information will be available on MyChart to the parent or guardian. If a minor patient over the age of 12 has developmental disabilities leading to diminished capacity, which has been confirmed by the patient’s physician, then the parent or guardian may have unrestricted access to such minor child’s MyChart. Any proxy access to a parent or guardian will terminate when a minor patient turns 18 years of age.

How to Request Proxy Access. Parents or guardians of minor patients or individuals 18 years or older may request proxy access to MyChart by completing a Proxy Access form. Proxy access to a competent adult’s MyChart will only be granted if the individual has consented to proxy access. If an adult is not competent to consent, then an adult with legal authority to consent for the adult patient may be authorized for proxy access. For proxy access to MyChart, the party seeking proxy access must be able to demonstrate the legal right to the patient’s medical information. If such a party’s legal right to the patient’s medical information changes, then they must immediately notify UI Health. UI Health reserves the right to revoke proxy access to a patient’s MyChart account for any reason.

User Name and Password.After signing up for MyChart, you will receive an activation code, which you will use to create a user name and password for your account. After you have used your activation code, it will no longer be valid. It is important to keep your user name and password private and confidential. Anyone with your user name and password can access your MyChart account and could potentially view your confidential health information. If you suspect your password has been compromised, it is your responsibility to change your password. You can update your password in MyChart at any time.

Confidentiality and Security.UI Health affords the same level of privacy and confidentiality to health information stored on MyChart as given to health information stored in any other medium. UI Health is committed to protecting the confidentiality of your health information, and we limit UI Health employees’ access to your information based on their role in your care. UI Health uses encryption, passwords, firewalls, and audit trails to protect the security of your information. In addition, UI Health will identify the records released and note the date and time that each patient accesses MyChart. UI Health has taken steps to make information received from our online visitors as secure as possible against unauthorized access and use.

For other than general information viewing, MyChart offers a high level of encryption technology commercially available and must be accessed with a Secure Socket Layer (SSL) compatible browser or terminal over HTTPS. You can confirm when you are secure by looking at the location (URL) field. If the URL field begins with https:// (instead of http://), the document comes from a secure server. You can also view the digital certificate to check whether you are connected to MyChart. This certificate verifies the connection between UI Health MyChart server’s public key and the server’s identification. User names and passwords provide two layers of authentication and are stored in an encrypted database separate from the Internet.

These Terms may be revised from time to time as UI Health adds new features and services, as laws or other applicable requirements change, and as industry privacy and security practices evolve. UI Health will take reasonable steps to notify you of material changes we make to these Terms. We display an effective date on the terms above so that it will be easier for you to know when there has been a change. You are responsible for regularly reviewing these Terms. Your continued use of MyChart constitutes your acceptance of any revised Terms and further indicates your agreement to abide by and be bound by those Terms. Small changes or changes that do not significantly affect individual privacy interests may be made at any time and without prior notice. If you do not agree to any modified Terms in the future, promptly exit MyChart and delete your account.
